Evaluation Form

Good Morning,

On Wed you said you will post the evaluation form.  As of today it's still not up.  I also sent you an email asking for form.

    How are we suppose to have this completed if you still have not sent nor posted the form for the class. If you are going to eliminate this part of the process, please inform the class.
